Telehealth Preventative Primary Care Advanced Practice Provider- NP/PA

Location: Fully remote within Ohio, with limited and occasional in-home visits to members in the local area

Salary: $115,000–$140,000 annually

What you will do

• Conduct primarily telehealth video visits and occasional local in-home visits.
• Perform comprehensive assessments and deliver evidence-based preventive care, chronic condition support, medication optimization, and health maintenance.
• Identify care gaps and coordinate with internal and external providers, pharmacists, nurses, caregivers, and families.
• Contribute to individualized, whole-person care plans for members with complex needs.
• Use integrated technology and AI-enabled workflows to reduce administrative work and improve member interaction.
• Communicate empathetically with members and caregivers to build trust.

Requirements

• Active Nurse Practitioner/APRN or Physician Assistant license.
• 3+ years of outpatient clinical practice, ideally in primary care, family medicine, internal medicine, or geriatrics.
• Active and clear NP/PA license, with willingness to obtain additional licenses within the first 90 days; support and reimbursement are provided.
• Active BLS certification at the time of hire.
• Comfort delivering care through video telehealth and using electronic documentation systems.

Nice to have

• Experience with managed care models, including STARS/HEDIS and care-gap identification.
• Experience providing comprehensive or preventive care to Medicare populations.
• Experience supporting older adults or people with complex medical or social needs.
