Telehealth Preventative Primary Care Advanced Practice Provider- NP/PA (Telehealth)

Location: Fully remote in South Carolina, with limited and occasional in-home visits for members in the local area

Salary: $115,000–$140,000 annually

What you will do

• Conduct primarily video-based telehealth visits, with limited occasional in-home visits for local members.
• Perform comprehensive assessments and deliver evidence-based preventative care, chronic condition support, medication optimization, and health maintenance.
• Identify care gaps and coordinate with primary care providers, specialists, pharmacists, nurses, care coordinators, caregivers, and families.
• Contribute to individualized, whole-person care plans for members with complex medical or social needs.
• Use integrated technology and AI-enabled workflows to reduce administrative work and increase meaningful member interaction.
• Communicate empathetically and clearly with members and caregivers.

Requirements

• Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.
• Three or more years of outpatient clinical practice, ideally in primary care, family medicine, internal medicine, or geriatrics.
• Active and clear NP/PA license; willingness to obtain additional licenses within the first 90 days, with support and reimbursement provided.
• Active BLS certification at the time of hire.
• Comfort delivering care through video telehealth and using electronic documentation systems.

Nice to have

• Familiarity with managed care models, including STARS/HEDIS and care-gap identification.
• Experience with comprehensive or preventative care visits for Medicare populations.
• Experience supporting older adults or people with complex medical or social needs.
