Salary: $115,000 - $140,000 annually

What you will do

• Conduct telehealth video visits and limited, occasional in-home visits to members in your local area.
• Perform comprehensive assessments focused on prevention, chronic condition support, and medication optimization.
• Identify and address care gaps while collaborating with an interdisciplinary clinical team.
• Utilize AI-enabled workflows to reduce administrative burden and maximize member interaction time.
• Develop long-term relationships with members to improve their health, dignity, and quality of life.

Requirements

• Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.
• Must be based in South Carolina.
• 3+ years of outpatient clinical practice in primary care, family medicine, internal medicine, or geriatrics.
• Active BLS certification at time of hire.
• Comfort with delivering care via video telehealth and using electronic documentation systems.

Nice to have

• Familiarity with managed care models, including STARS/HEDIS.
• Experience performing preventive care visits with Medicare populations.
• Experience supporting older adults and individuals with complex medical or social needs.
