Bilingual Care Partner (Spanish/English)

Location: Remote; requires a quiet, secure workspace with reliable internet and privacy during scheduled shifts.

What you will do

• Engage members by phone, text, and email throughout their cancer care journey.
• Assess health and social needs, identify urgency and root causes, and determine appropriate next steps.
• Connect members with nurses, providers, telehealth visits, diagnostic appointments, treatments, and community resources.
• Research and coordinate transportation, financial grants, emotional support, insurance benefits, and social services.
• Support advanced care planning and escalate clinical symptoms to the appropriate clinical teams.
• Document interactions, manage appointment tracking, and meet defined call volume, responsiveness, documentation, and engagement metrics.

Requirements

• At least 2 years of experience in a patient-facing role involving care coordination, healthcare navigation, non-clinical case management, resource navigation, or community health work.
• Spanish/English bilingual proficiency is required.
• Experience in a fast-paced, high-volume, metrics-driven environment such as a contact center, care navigation, or care coordination setting.
• Ability to build trust quickly, identify expressed and unexpressed needs, and communicate concisely with members and healthcare providers.
• Strong organization, adaptability, empathy, listening, and problem-solving skills.
• Comfort using or learning video chat, Google Suite, Slack, electronic health records, and other technology.

• Healthcare experience.
• Experience working in scaling businesses, fast-paced environments, or startups.
• Previous remote work experience.
