Bilingual Case Management Nurse

What you will do

• Coordinate healthcare services and communication among providers to deliver comprehensive, patient-centered care.
• Assess medical needs and develop individualized care plans aligned with patient conditions and health goals.
• Advocate for patients, support access to appropriate services, and help them navigate the healthcare system.
• Monitor progress, treatment-plan adherence, intervention effectiveness, and care outcomes.
• Document assessments, care plans, reports, and outcomes accurately and on time.
• Educate patients and families on medications, treatment plans, self-care, preventive care, and wellness.

Requirements

• Active, unrestricted Registered Nurse license in the state of residence; a Compact License is accepted for residents of compact states.
• At least 3 years of nursing experience in an acute care setting.
• Strong critical thinking, sound judgment, attention to detail, organization, and ability to work in a deadline-driven environment.
• Excellent communication and interpersonal skills with a collaborative approach to care coordination.
• Must be US based and able to comply with company policies, including HIPAA requirements.
• Quiet, distraction-free workspace, reliable internet, and adherence to security protocols using VPNs, secure passwords, and approved devices and software.

Nice to have

• Compact State Nursing License.
• Case management experience.
• Bilingual Spanish proficiency.
• Certified Case Manager, Chronic Care Professional, or willingness to obtain CCM certification within 18 months of hire.
