Bilingual (Spanish/English) Advanced Practice Provider: Complex Geriatric Care

Salary: $125,000–$155,000 per year

What you will do

• Deliver video telehealth care to a cohort of complex patients across multiple geographies.
• Guide and oversee medical management using the 5M framework, including symptom and chronic disease management.
• Perform comprehensive assessments, diagnosis and disease reviews, medication reviews, post-acute transition support, and quality-of-care interventions.
• Coordinate care with primary care providers, specialists, pharmacy, specialty teams, family members, caregivers, and interdisciplinary colleagues.
• Assess social needs, home health requirements, and durable medical equipment needs while maintaining accurate medical records.
• Provide occasional weekend on-call support or telehealth coverage for other Devoted Medical service lines when appropriate.

Requirements

• Maintain an active, clear RN and APRN or Physician Assistant license, with willingness to obtain and maintain multiple state licenses.
• All required state licenses must be in place by the start date; specific states will be confirmed at the offer stage.
• Three or more years of APP experience in primary care, house calls, internal medicine, family medicine, geriatrics, and/or palliative care.
• Proficiency with telehealth technology and electronic health records.
• Active BLS certification at the time of hire and throughout employment.
• English is required; Spanish proficiency is preferred but not required.

• Virtual care experience and interest in continuing clinical practice through virtual visits.
• Experience collaborating with interdisciplinary teams and caring for complex, frail, or vulnerable populations.
• Experience in a Skilled Nursing Facility setting.
