What you will do

• Deliver exceptional care to complex patients across multiple geographies using video telehealth visits.
• Guide medical management of patients utilizing the 5M framework.
• Collaborate with PCPs, specialists, pharmacy teams, and patient caregivers to coordinate care plans.
• Perform comprehensive assessment visits, post-acute transition support, and medication reviews.
• Maintain accurate patient medical records in compliance with legal and ethical guidelines.
• Provide occasional weekend on-call support for clinical nurses during triage.

Requirements

• Active and clear RN and APRN or Physician Assistant license.
• Must be based in the USA and willing to obtain multiple state licenses.
• 3+ years of experience as an APP in primary care, internal medicine, family medicine, geriatrics, or palliative care.
• Proficiency in telehealth technology and electronic health records (EHR).
• Active BLS certification at time of hire.

Nice to have

• Experience in virtual care and practicing clinical nursing in a remote setting.
• Experience working with complex or vulnerable populations in Skilled Nursing Facility (SNF) settings.
• Proficiency in Spanish.
• Experience collaborating within interdisciplinary teams.
