Location: Remote Illinois; must reside in the state of Illinois. Field position requiring 75% local travel to members' homes, assisted living facilities, long-term care facilities, and other care settings. Occasional travel to hirify.global offices for training or meetings may be required. Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m. CST, with flexibility based on business needs.

Salary: $53,700–$72,600 per year.

What you will do

  • Proactively contact newly enrolled Illinois Medicaid members and complete comprehensive health risk assessments.
  • Assess member needs through telephonic and face-to-face interactions, identifying healthcare, social service, and immediate care gaps.
  • Plan, implement, monitor, and evaluate care management interventions against members' personal goals.
  • Coordinate services with providers, payer sources, interdisciplinary teams, families, and community resources.
  • Help members file grievances and appeals and explain managed care organization requirements, services, and benefits.
  • Support members in accessing appropriate care settings and resolving barriers to effective care.

Requirements

  • Bachelor's degree in social sciences, social work, human services, or a related field, or an unrestricted Illinois LPN license with one year of relevant assessment and elderly-care experience, or an LPC or LSW license.
  • Residence in Illinois is required.
  • At least one year of healthcare and/or case management experience.
  • At least one year of experience with Medicare or Medicaid recipients, long-term care, home and community-based services, or managed care organizations.
  • Intermediate to advanced computer skills with Microsoft Word, Excel, and Outlook, plus experience with electronic medical records and other information systems.
  • A valid state driver's license and personal vehicle liability insurance are required. Patient-facing work also requires TB screening.
