Nurse Practitioner (Healthcare)

What you will do

• Conduct virtual adult and pediatric wellness visits and health assessments for patients in their homes.
• Perform health risk assessments, medication reviews, cognitive screenings, and patient education.
• Collaborate with in-home clinical staff to evaluate vital signs, arrange blood draws, and address patient needs.
• Provide care coordination and management, including preventive interventions, medication management, specialist referrals, and community resources.
• Document clinical findings and coordinate care using laptops, mobile devices, and HIPAA-secure messaging tools.

Requirements

• Board certification as a Family Nurse Practitioner.
• Active Family Nurse Practitioner license in KY, MI, OH, or IL.
• Professionalism, integrity, accountability, confidentiality, compassion, and commitment to high-quality care.
• Strong written and verbal communication skills and the ability to work independently or collaboratively.
• Technological proficiency with mobile devices, charting tools, and secure messaging applications.
• Successful completion of a national background check and clinical license search.
