Nurse Practitioner

Location: Remote, United States; active PA licensure required, with the ability to obtain additional state licenses.

Salary: $75/hour regular rate and $85/hour premium rate.

What you will do

• Provide phone- and video-based transitional care to patients after hospital discharge.
• Conduct telephonic outreach and scheduled visits for symptom screening and follow-up.
• Perform medication reconciliation and order necessary labs, medications, and medical supplies.
• Build patient trust, explain the care model, and connect patients with appropriate services.
• Coordinate care across hospitals, emergency departments, pharmacies, and community providers.
• Collaborate with nursing and administrative colleagues while meeting individual and population health goals.

Requirements

• Certified Nurse Practitioner in adult, family, or geriatric practice.
• Active Pennsylvania state license required, with eligibility to obtain additional state licenses.
• 5+ years of post-graduate clinical experience in primary care or geriatrics; up to two years may be substituted with skilled-nursing or long-term-care experience.
• Experience caring for Medicare and Medicaid populations with complex, comorbid conditions.
• Motivational interviewing, strong communication, collaboration, problem-solving, and technology skills.
• Availability for 24–36 hours per week, including monthly weekend shifts and four-hour shifts across weekdays and weekends.

Nice to have

• Active licenses in Texas, New Jersey, Illinois, New York, Massachusetts, Minnesota, Louisiana, or South Carolina.
• Bilingual English and Spanish communication skills with cultural fluency.
• Familiarity with Google Suite and Slack.
