Field Care Manager - RN

Location: Must reside in Cook County, Illinois, within the specified ZIP codes or within a 10-mile radius; requires 75–90% travel across Illinois and occasional onsite work at the Schaumburg market office. Monday–Friday, 8:00 a.m.–5:00 p.m. CST.

Salary: $78,200–$107,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members’ physical, environmental, and psychosocial health needs.
• Create and manage individualized plans of care.
• Identify and resolve barriers to effective care and monitor progress toward desired outcomes.
• Visit members in homes, supportive living facilities, long-term care facilities, and other care settings.
• Coordinate with internal and external stakeholders and connect members with community resources.

Requirements

• Must reside in the specified Cook County, Illinois ZIP codes or within a 10-mile radius.
• Unrestricted Illinois Registered Nurse license with no disciplinary action.
• Clinical nursing experience in hospital, acute care, or home health settings.
• Ability to travel 75–90% within Illinois.
• Knowledge of community health, social service agencies, and community resources.
• Strong communication and interpersonal skills, intermediate to advanced computer skills, and proficiency with Word, Outlook, and Excel.

Nice to have

• BSN and two or more years of in-home case or care management experience.
• Medicare/Medicaid, managed care, health promotion, coaching, or wellness experience.
• Case Management or Motivational Interviewing certification.
• Bilingual English and Spanish skills.
