RN Case Manager, Transition of Care (TOC) - PG County - Up to $5,000 sign on bonus

Location: Prince George’s County, United States; member visits may take place in homes, skilled nursing facilities, inpatient rehabilitation facilities, shelters, and hospitals.

Salary: $71,000–$90,500 annual; eligible for up to a $5,000 sign-on bonus.

What you will do

• Coordinate Transition of Care assignments and outreach with hospital case managers.
• Complete member intake, self-efficacy assessments, condition-specific screeners, and appropriate clinical exams.
• Participate in inpatient rounds and post-discharge case conferences to support discharge planning.
• Develop post-discharge care plans with care coordinators and behavioral health specialists.
• Conduct home visits and weekly follow-ups for four weeks, addressing clinical and non-clinical needs.
• Perform medication reconciliation, document care, close quality gaps, and track Transition of Care metrics.

Requirements

• Graduate of an accredited school of nursing.
• Registered Nurse (R.N.) qualification.
• 3+ years of professional experience.
• Ability to provide member-facing care in community and healthcare settings.
• Medical clearance is required, including evidence of immunity to MMR, Hepatitis B, and Varicella, plus a TB screen, unless an approved medical or religious accommodation applies.
