Salary: $22–$25 per hour, with compensation listed at $25 per hour

What you will do

  • Manage a caseload of patients with complex medical and social needs, complete assessments, and develop and implement care plans.
  • Build rapport with patients and coach them on behavioral changes and risk reduction for common health conditions.
  • Accompany members to medical and social service appointments and connect them with appropriate programs.
  • Link patients to local, county, and state services while providing follow-up and advocacy.
  • Support self-management, engage eligible patients in the Akido ECM Program, and document every encounter in case management systems.

Requirements

  • High school diploma or equivalent.
  • 2+ years of experience working with CalAIM populations of focus, including people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, or adults with complex medical needs.
  • Ability to work independently and as part of a team in a dynamic outdoor environment.
  • Strong oral communication and interpersonal skills, including the ability to articulate relevant personal experiences.
  • Ability to use computers and case management software for documentation.
  • Valid driver’s license, automobile insurance, reliable transportation, and availability for 50–75% travel required.

Nice to have

  • Bilingual English and Spanish communication skills strongly preferred.
