Senior Backend Software Engineer (Healthcare AI)

Location: Hybrid in the New York City metro area, San Francisco Bay Area, or Los Angeles; office attendance required 2 days per week, with more frequent in-office work during onboarding and team onsite weeks. Relocation assistance is available for candidates who do not already reside in the NYC metro area. Candidates must be authorized to work in the US without visa sponsorship.

Salary: $165,000–$220,000 per year, plus equity.

What you will do

• Design, develop, deploy, and iterate on critical backend services for the Diagnostics product.
• Own improvements across system design, observability, testing, performance, and production support.
• Model clinical data and design schemas and service contracts for new product surfaces.
• Work with clinicians to translate diagnostic requirements into extensible logic and tooling.
• Make architectural decisions, lead major projects, mentor engineers, and review backend code.
• Participate in on-call operations, monitoring, and production support.

Requirements

• 6+ years of professional software development experience and a BS in Computer Science or equivalent experience.
• Proficiency in Python or Go, with experience maintaining production-quality code.
• Experience building data-intensive applications and designing and evolving schemas used across cross-functional teams.
• Ability to participate in on-call operational support.
• Practical experience using AI coding tools such as Cursor, Claude Code, or Codex.
• US work authorization without visa sponsorship and availability for hybrid work in the NYC metro area, San Francisco Bay Area, or Los Angeles.

Nice to have

• Experience with clinical or healthcare data, including FHIR, EMR integrations, clinical terminologies, or revenue cycle systems.
• Experience evaluating ranked, scored, or probabilistic outputs against imperfect ground truth.
• Startup or high-growth experience and experience building tools for non-engineer domain experts.
• Experience managing cloud infrastructure, LLM integrations, or mature enterprise SaaS products.
