Nurse Practitioner OR Physicians Assistant (NP/PA) - Hybrid (Cincinnati OR Columbus)

Location: Remote from Cincinnati or Columbus, Ohio, with visits to a hub location 1–2 times per week

Salary: $125,000–$135,000 per year

What you will do

• Conduct comprehensive clinical intake visits and provide longitudinal primary or wrap-around care.
• Manage chronic conditions including hypertension, diabetes, and chronic kidney disease, while addressing acute concerns and preventive care needs.
• Manage behavioral health conditions, including depression, anxiety, alcohol use disorder, and opioid use disorder, including prescribing first- and second-line medications.
• Conduct member outreach, follow-up after care transitions or acute needs, and support progress toward physical, behavioral, and social health goals.
• Document diagnoses and encounters accurately, including HCC, ICD-10, and CPT coding to support quality-gap closure.
• Participate in case conferences and collaborate with interdisciplinary teams on care plans and high-impact clinical interventions.

Requirements

• At least 3 years of direct clinical care experience.
• Completion of an accredited Nurse Practitioner or Physician Assistant program with national certification.
• Current, valid, unrestricted license to practice in the state of employment with hirify.global.
• Physician Assistants must hold NCCPA certification; Nurse Practitioners must hold an unrestricted state license.
• DEA licensure or willingness to acquire it.
• Certification in Basic Life Support for Healthcare Providers.
