Central Pre-Enrollment Provider Nurse Practitioner

Location: Remote within the United States; California NP or PA licensure and U.S. work authorization required

Salary: $133,000–$152,000 annually, plus a target annual bonus of 10%

What you will do

• Conduct complete chart reviews for PACE applicants, evaluating medical records, diagnostic history, functional status, and clinical documentation.
• Assess PACE eligibility and Health & Safety considerations, including virtual clinical assessments when diagnostic gaps cannot be resolved.
• Present complex cases to Site Medical Directors and participate in Health & Safety Committee meetings.
• Collaborate with Enrollment RNs and Site Medical Directors to resolve documentation gaps before submission.
• Review ILOC clinical summaries for connections between medical conditions, functional limitations, and PACE services.
• Support quality improvement initiatives focused on DHCS approval rates and documentation accuracy.

Requirements

• Current, unrestricted California Nurse Practitioner or Physician Assistant license required.
• Current California DEA certification and furnishing or prescriptive authority required.
• At least three years of post-licensure clinical experience in geriatrics, long-term care, or a PACE-adjacent setting.
• Experience with chart review, geriatric assessment tools, ILOC documentation, and CMS/DHCS PACE eligibility and NFLOC requirements.
• Proficiency with Epic EHR and Microsoft Outlook, including high-volume, time-sensitive communication.
• Experience collaborating with physicians, RNs, and social workers and presenting complex cases to Medical Directors.

Nice to have

• Previous PACE experience or experience serving older adults with complex needs.
• Telehealth and virtual assessment experience, including Adult-Gerontology Nurse Practitioner certification or active pursuit of certification.
• Familiarity with DHCS ILOC submissions, Health & Safety committees, geriatric behavioral health, or care transitions.
• Bilingual Spanish and English skills.
