Nurse Practitioner or Physician Assistant (Virtual Health Assessment)

Location: Remote for candidates residing in Arizona, Arkansas, California, Colorado, District of Columbia, Florida, Georgia, Illinois, Massachusetts, Michigan, Nevada, New Hampshire, New Jersey, New York, North Carolina, Ohio, Pennsylvania, Texas, or Virginia. Occasional travel may be required for team meetings and company events. Work is scheduled during EST business hours: 8 a.m.–8 p.m. Monday–Thursday or 8 a.m.–6 p.m. Friday–Saturday, with one Saturday shift per quarter.

Salary: $130,000–$140,000 plus annual bonus.

What you will do

• Conduct virtual health assessment, evaluation, and management visits for hirify.global members.
• Perform preventive care screenings, medication reconciliation, diagnosis, treatment, referrals, and health coaching.
• Conduct outreach to members with chronic or suspected health conditions and those less likely to see a primary care provider.
• Identify opportunities to improve members’ access to care through licensed virtual providers.
• Cross-train to support virtual urgent care and virtual primary care services.
• Maintain compliance with applicable laws and regulations.

Requirements

• PA, DNP, FNP, or ENP qualification from an accredited program.
• Board certification through AANP, ANCC, or NCCPA.
• Compact RN license for Nurse Practitioner candidates.
• At least 3 years of experience in outpatient family medicine, internal medicine, or primary care.
• Experience with Clinical Documentation Improvement and/or HEDIS quality measures.
• Required state licensure varies by discipline: PAs must be licensed in Georgia and Florida, Texas, or Ohio; NPs must be licensed in Florida and Texas or Ohio, with the ability to obtain additional licenses.
