Ambulatory Care Clinical Pharmacist - OH, MI, IL (MUST hold a license in one of these states) - REMOTE

Location: Remote within the USA; active pharmacist licensure in at least one of Michigan, Illinois, or Ohio is required. Ohio licensure must be obtained upon hire, with company support and reimbursement.

Salary: $140,000–$150,000 annually

What you will do

• Provide Comprehensive Medication Management and pharmacist support services through hub-based and virtual clinical care teams.
• Develop medication management action plans with measurable clinical outcomes, monitoring parameters, and follow-up timelines.
• Collaborate with internal and external providers, nurse care managers, community health partners, and other care team members.
• Identify and resolve medication therapy opportunities while designing individualized regimens for chronic disease management and preventative care.
• Provide drug information, dosing, interaction, adverse-effect, and member-specific support to clinical teams.
• Develop medication management pathways and protocols supporting future collaborative practice agreements.

Requirements

• Active pharmacist license in at least one of Michigan, Illinois, or Ohio.
• Eligibility and willingness to obtain Ohio pharmacist licensure upon hire.
• PGY1 pharmacy practice residency and PGY2 ambulatory care pharmacy residency, or at least two years of clinical pharmacist experience in an ambulatory care setting.
• Ability to provide direct member and caregiver care and collaborate with multidisciplinary clinical teams.
• Ability to complete medical clearance requirements, including evidence of immunity to MMR, Hepatitis B, and Varicella, plus a TB screen, or obtain an approved accommodation.
