What you will do

  • Conduct comprehensive initial post-discharge visits, including medication reconciliation and assessment of psychosocial needs.
  • Perform acute virtual urgent-care visits to diagnose and manage new or worsening symptoms in medically complex patients.
  • Manage clinical trajectories over a 30-day window, adjusting care plans based on evolving clinical pictures.
  • Collaborate with an interdisciplinary team including RN case managers, social workers, and physicians.
  • Contribute to program development, quality-improvement projects, and EHR feedback.

Requirements

  • 5+ years of direct patient care managing geriatric populations with complex, comorbid conditions.
  • Proficiency in English and Spanish is required.
  • Active and in-good-standing APRN or PA license (must be willing to obtain licenses in FL, TX, AZ, OH, and GA).
  • Ability to work a full-time 5-day, 8-hour schedule within CST, MST, or PST time zones.
  • Strong clinical decision-making skills for both acute and longitudinal chronic disease management.
