Care Manager, Behavioral Health

Location: Remote nationwide within the United States; candidates currently living in Puerto Rico cannot be hired. U.S. citizenship and interim approval for a government security clearance are required. Work hours must include an 8-hour shift between 8:00 a.m. and 6:00 p.m. EST; mandatory training runs from 8:00 a.m. to 5:00 p.m. EST for the first four weeks. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $59,300–$80,900 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess, plan, coordinate, implement, monitor, and evaluate the care needs of TRICARE beneficiaries by phone.
• Serve as the primary point of contact and care coordinator across the beneficiary’s clinical spectrum.
• Develop integrated care plans addressing behavioral health and other beneficiary needs and conditions.
• Collaborate with Military Treatment Facilities, physicians, providers, and internal care management programs.
• Support beneficiaries’ self-care capacity, quality of life, and desired clinical outcomes.

Requirements

• U.S. citizenship is required.
• Interim government security clearance approval through NBIS is required.
• A current, valid, unrestricted behavioral or mental health professional license in the state of residency is required, such as LCSW, LPC, LMHC, LMSW, or Clinical Psychologist (Ph.D.).
• At least three years of clinical experience, including possible managed care experience.
• Experience managing mental health crisis intervention.
• Ability to obtain Certified Care Manager certification within the first year of employment.

Nice to have

• Active Certified Care Manager certification.
• Experience with acute inpatient hospital case management.
• Experience with medical or behavioral health case management programs.
• Experience with TRICARE policies or the military healthcare delivery system.
