Physician, Bay Area Street Medicine Focus

Location: San Francisco Bay Area, California, United States. Field-based work with up to 75% travel, including substantial walking, standing, driving, and care delivery in outdoor and non-traditional settings.

Salary range: $240,000–$350,000 USD per year.

What you will do

• Provide frontline acute, chronic, preventive, and palliative care to people experiencing homelessness and patients with complex medical, behavioral health, substance use, and incarceration-transition needs.
• Conduct assessments, take medical histories, perform examinations, order and interpret diagnostics, and prescribe treatment within California scope of practice.
• Develop care plans supporting patient stability, improved outcomes, and symptom management across community-based settings.
• Coordinate care with nurses, social workers, outreach staff, and other multidisciplinary professionals.
• Provide clinical mentorship and guidance to care team members.
• Document encounters accurately to maintain continuity of care.

Requirements

• MD or DO degree with an active, unrestricted California Physician & Surgeon license.
• At least 2 years of clinical experience caring for underserved populations, including unhoused adults, adults with SMI/SUD, people transitioning from incarceration, or patients with complex medical needs.
• Experience in street medicine or related community-based settings such as shelters and outreach programs.
• Board certification or eligibility, DEA registration or eligibility to obtain it, NPI number, and BLS certification.
• Ability to provide care independently in dynamic field conditions and collaborate with a multidisciplinary team.
• Valid California Driver’s License and access to a reliable, insured vehicle; ability to travel up to 75% of work time.

Nice to have

• Familiarity with Bay Area medical, social, and community resources, neighborhoods, transit, and community partners.
• Experience in family medicine, internal medicine, emergency medicine, palliative care, geriatrics, addiction medicine, or pain management.
• Experience with value-based care models and team-based practice.
• Bilingual abilities in Spanish or another language.
