Resource Specialist Care Manager (Pediatrics)

Location: United States (Remote); candidates must be licensed in NY, NJ, NC, GA, FL, TX, MO, TN, or Washington DC. The role includes 10% travel for offsite meetings, training, and educational purposes. Working hours are 8:00am-5:00pm EST Monday-Friday, with two days set with the supervisor from 10:00am-7:00pm EST.

Salary: $70,000-$77,000 annually, plus benefits and potential participation in an employee equity purchase program.

What you will do

• Assess the medical, behavioral, and complex social needs of children and their families.
• Identify barriers such as food insecurity, unstable housing, transportation challenges, and insurance limitations.
• Collaborate with families and interdisciplinary teams to establish care goals and maintain integrated care plans.
• Coordinate referrals, evaluations, treatment recommendations, and follow-up activities.
• Educate families about local resources and agencies, provide emotional support, and advocate for access to care.
• Build safety plans, maintain community partnerships, monitor patient progress, and represent the organization at community events and resource meetings.

Requirements

• Master's degree in social work.
• LMSW or comparable state licensure required.
• 2-3+ years of postgraduate experience in social service case management.
• At least 1 year of experience working with pediatric populations.
• Knowledge and application of motivational interviewing practices or techniques.
• Experience providing telehealth services and engaging families in a remote environment.

Nice to have

• Experience working with acute populations and familiarity with school and community resources.
• Bilingual Spanish proficiency.
