Manager of Care Management, Health Homes (NY / Hybrid)

Location: New York, NY; hybrid work with field travel required when needed

Salary: $100,000–$130,000 per year

What you will do

• Build and operationalize a Health Home Care Management Program with the VP of Operations, Team Lead, and Health Home leads.
• Develop workflows and processes for program policies and procedures.
• Recruit, onboard, train, supervise, schedule, and provide performance feedback to care management staff.
• Review assessments, documentation, care plans, patient charts, and billing for compliance with Health Home requirements.
• Establish quality improvement, quality assurance, compliance goals, and reporting processes.
• Guide staff through patient crises, de-escalation, incident reporting, and field coverage when needed.

Requirements

• Master’s degree or a professional occupational or healthcare license, such as a Registered Nurse license.
• LMSW or MSW with prior management experience in a behavioral or healthcare setting.
• At least 3 years of supervisory experience in the Health Homes field and care management experience.
• Ability to travel in the field, meet patients in person, accompany patients to appointments, and respond to escalated situations.
• Knowledge of local community resources and experience with data reporting, documentation, compliance, and outcomes.
• Strong communication, assessment, empathy, crisis-management, and relationship-building skills; experience with Netsmart, Foothold, and Salesforce.

Nice to have

• Bilingual Spanish skills.
• 5+ years of experience as a Care Manager in the New York State Health Homes program.
