Case Manager, ECM - Skid Row

Location: Los Angeles, CA; minimum 75% travel required for field-based work in Skid Row.

Compensation: $28/hour USD

What you will do

• Manage a caseload of patients with complex medical and social needs, including assessments, care plans, and follow-up activities.
• Conduct street outreach and engage eligible patients for enrollment in the Akido ECM Program.
• Build rapport, provide health education and coaching, and support positive behavior changes and self-management.
• Accompany patients to medical and social-service appointments and connect them with appropriate programs.
• Link patients with local, county, and state services while serving as their advocate.
• Document every patient encounter in case management systems.

Requirements

• High school diploma or equivalent.
• At least 2 years of experience working with CalAIM populations, including people experiencing homelessness or adults with complex medical needs.
• Valid driver’s license, automobile insurance, and reliable transportation required.
• Ability to work independently and collaboratively, prioritize competing tasks, and communicate effectively.
• Strong oral communication and interpersonal skills.
• Ability to use computers and case management software for documentation.
• Ability to work in a dynamic outdoor environment and provide street outreach in Skid Row.
