Lead Physician (MD/DO/NP/PA) (Savannah GA) - Hybrid

Location: Hybrid role based in Georgia, with in-home visits across Albany, Thomasville, and Macon. Relocation support is available for exceptional providers moving to Georgia. The role includes in-home and telehealth visits.

Base salary: $220,000–$260,000 per year. The posting also lists a range of $220,000–$240,000 USD.

What you will do

• Oversee care and outcomes for a patient cohort, managing cases directly and alongside nurse practitioners.
• Conduct in-home and telehealth visits for Medicare Advantage members using Clover Assistant and the EMR.
• Coordinate with interdisciplinary care teams and other providers to develop personalized care plans and close gaps in care.
• Provide clinical leadership and mentorship to nurse practitioners.
• Lead quality improvement initiatives and assurance reviews.
• Collaborate with the Medical Director and Practice Manager to improve care processes and performance.

Requirements

• MD or DO degree with board certification in Internal Medicine, Family Medicine, or Geriatric Medicine.
• Minimum five years of advanced clinical practice experience.
• Expertise in chronic disease management and care for vulnerable senior populations.
• Georgia medical licensure, or willingness to obtain it; DEA and CDS registrations are required, with support available for obtaining them.
• Interest in using technology to improve clinical care and member outcomes.
