Medical Director (PACE)

Location: 4659 S Cottage Grove Ave, Chicago, IL 60653, United States; local and regional travel may be required.

Salary: $290,000–$350,000 annual base salary, plus a target annual bonus of 20% and potential additional incentive compensation.

What you will do

• Provide evidence-based, person-centered geriatric primary care for an approximately 50% clinical panel and serve as supervising physician.
• Coordinate care planning across medical, behavioral, and functional needs with the interdisciplinary team.
• Lead clinical quality, safety, outcomes, participant experience, medication management, and documentation integrity for the PACE center.
• Chair the Quality Improvement Committee, analyze utilization and safety data, and lead root cause analyses and corrective actions.
• Oversee physicians and advanced practice providers while providing coaching, mentorship, and case-based education to interdisciplinary staff.
• Partner with the Executive Director and external healthcare providers on center performance, care delivery, and sustainable panel growth.

Requirements

• MD or DO with active, unrestricted state licensure, DEA license, and current board certification in Internal Medicine, Family Medicine, Geriatrics, or a relevant specialty.
• At least 3 years of clinical experience caring for older adults or medically complex populations in PACE, geriatrics, SNF, hospice, home-based care, managed care, or a comparable integrated-care setting.
• Demonstrated ability to provide direct clinical care for complex seniors and manage care across clinic, home, inpatient, and SNF settings.
• Experience leading clinicians and interdisciplinary teams through coaching, mentorship, and case-based teaching.
• Experience with value-based or capitated care models and using clinical, utilization, and quality data to improve outcomes.

Nice to have

• Previous Medical Director, Associate Medical Director, or comparable physician leadership experience in PACE, integrated senior care, or value-based care.
• Experience with HCC coding, documentation improvement, chart review, audit readiness, QAPI, quality, utilization, or safety initiatives.
• Experience serving as a physician liaison with health system partners or referral networks, and familiarity with CMS PACE regulations and the 11-discipline IDT model.
