Complex Care Physician (AI)

Salary: $210,000–$315,000 per year

What you will do

• Deliver virtual care to high-risk patients through an AI-enabled platform, video telehealth visits, and interdisciplinary teams.
• Manage complex geriatric conditions, including polypharmacy, falls, and dementia, using the 5M framework.
• Lead clinical performance management by monitoring outcomes, analyzing data, and optimizing team metrics.
• Improve the clinical model and identify ways to use AI to advance clinical outcomes.
• Coordinate care with patients, caregivers, PCPs, specialists, pharmacy, disease management, education, social work, and palliative teams.
• Use the Orinoco electronic health information system and provide feedback to improve its interface.

Requirements

• Board certification as a geriatrician, primary care, internal medicine, or family medicine physician.
• At least two years of clinical experience.
• Experience providing care in collaboration with interdisciplinary teams.
• Ability to work remotely from the USA.
