What you will do
- Deliver virtual care via video telehealth and AI platform to complex patients across geographies with interdisciplinary support.
- Clinically manage high-risk geriatric cases focusing on polypharmacy, falls, dementia, and transitions from hospital to home.
- Monitor team performance metrics, trend outcomes, and collaborate with leaders to meet clinical targets.
- Assess and iterate on clinical models, leveraging AI for better outcomes and acting as a change agent.
- Engage patients, caregivers, PCPs, specialists, and internal teams like pharmacy, social work, and palliative care for comprehensive care.
- Use electronic health system (Orinoco) for visits and provide feedback for improvements while stewarding patient-centric culture.
Requirements
- Board Certified in Geriatrics, Primary Care, Internal Medicine, or Family Medicine with at least 2 years clinical experience.
- Experience providing clinical care in collaboration with interdisciplinary teams.
- Must be eligible to work in the USA (inferred from Remote USA and US benefits).

- Experience with complex, frail, or vulnerable populations, including SNF settings.
- Virtual care experience.
