Salary: $210,000–$315,000 / year

What you will do

• Deliver virtual care via video telehealth and AI platform to complex patients across geographies with interdisciplinary support.
• Clinically manage high-risk geriatric cases focusing on polypharmacy, falls, dementia, and transitions from hospital to home.
• Monitor team performance metrics, trend outcomes, and collaborate with leaders to meet clinical targets.
• Assess and iterate on clinical models, leveraging AI for better outcomes and acting as a change agent.
• Engage patients, caregivers, PCPs, specialists, and internal teams like pharmacy, social work, and palliative care for comprehensive care.
• Use electronic health system (Orinoco) for visits and provide feedback for improvements while stewarding patient-centric culture.

Requirements

• Board Certified in Geriatrics, Primary Care, Internal Medicine, or Family Medicine with at least 2 years clinical experience.
• Experience providing clinical care in collaboration with interdisciplinary teams.
• Must be eligible to work in the USA (inferred from Remote USA and US benefits).

• Experience with complex, frail, or vulnerable populations, including SNF settings.
