LICSW, Clinic Therapist

Location: Hybrid role split between the Crystal and Rosedale clinics in Minnesota and remote work from home.

Salary: $80,000+ base salary, dependent on experience.

hirify.global is building a specialized primary care model for women aged 65+, addressing post-menopausal care, bone density, weight management, and mental, social, and emotional well-being.

What you will do

• Provide counseling, crisis intervention, grief counseling, stress management, and psychosocial support to patients and families.
• Coordinate medical, social, and emotional care plans with providers, patients, and families.
• Assess social, emotional, and environmental needs, including social determinants of health.
• Address medication adherence barriers and connect patients with community resources such as housing, financial, food, and mental health services.
• Provide telephone and remote support, patient advocacy, health education, and follow-up reinforcement.
• Document care and monitor patient outcomes in Athena EMR while maintaining confidentiality and ethical standards.

Requirements

• Active, unrestricted LICSW license in Minnesota required.
• 2+ years of experience supporting patients in a counseling setting.
• Behavioral health and telephone-based patient engagement experience.
• Knowledge of community resources and programs in the Twin Cities.
• Experience using and documenting in an EMR system; proficiency with web-based technology and Microsoft Office.
• Reliable internet and a home office suitable for discussions of sensitive health information required.

• Certified Case Manager credential.
