Social Worker (MSW)

Location: Onsite in Sacramento, California, United States

Salary: $74,000–$84,000 per year, plus possible incentive compensation

hirify.global provides integrated medical and social care for older adults through the Program of All-Inclusive Care for the Elderly (PACE), including support with daily needs and in-home services.

What you will do

• Collaborate with the Interdisciplinary Team to optimize participants’ health status and quality of life.
• Conduct social work assessments to identify psychosocial needs, preferences, and goals.
• Deliver and document care-plan interventions, including resource coordination, service arrangements, counseling, and case management.
• Coordinate care transitions between hospitals, skilled nursing facilities, homes, and other settings.
• Initiate and facilitate care conferences with care teams, participants, families, and informal caregivers.
• Provide discharge planning when participants leave the program.

Requirements

• Master’s degree in Social Work (MSW).
• At least one year of experience working with frail or elderly people.
• Experience coordinating and facilitating care conference meetings.
• Medical clearance for communicable diseases, including a TB test, and proof of current immunizations.
• Current CPR/BLS certification or the ability to obtain it within 30 days of employment.
• Valid California driver’s license, personal transportation, a good driving record, auto insurance, and authorization to work in the United States are required.

Nice to have

• Bilingual Spanish/English communication skills.
• Experience supporting patients with behavioral health or substance abuse issues.
• Experience with PACE, ADHC, or CBAS programs.
