What you will do

• Provide skilled nursing visits in patients’ homes under physician-directed plans of care.
• Conduct initial and ongoing assessments using OASIS data elements.
• Formulate, implement, evaluate, and adjust individualized care plans with patients, caregivers, and physicians.
• Monitor patients’ conditions, provide comfort and safety, transcribe physician orders, and report unusual reactions or changes.
• Document observations and treatments, prepare service summaries and progress-report information, and participate in case conferences and improvement activities.
• Supervise Licensed Practical Nurses and Home Health Aides when assigned.

Requirements

• Graduate of an approved school of nursing.
• Current nursing license in the State of Texas required.
• At least one year of current healthcare experience.
• Home health experience preferred.
• Excellent verbal communication skills.
• Ability to follow agency policies and procedures, including HIPAA privacy and confidentiality requirements.
