Field Care Coordinator- Bilingual Preferred (Healthcare)

Location: Must reside in Hillsborough County, Florida; the role includes 75–90% local travel to members’ homes, assisted living facilities, long-term care facilities, and other care settings.

Starting salary: $60,000–$65,000 per year based on qualifications and experience. Posted pay range: $53,700–$72,600 per year.

What you will do

• Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings.
• Assess member needs, barriers, and requirements to create individualized care plans.
• Plan and implement interventions that support optimal wellness and the least restrictive care setting.
• Coordinate services with payers, providers, interdisciplinary teams, families, and community resources.
• Monitor and evaluate care plans against members’ personal goals and health outcomes.
• Provide health education, encouragement, and guidance to members and families.

Requirements

• Bachelor’s degree in a health and human services field.
• At least 2 years of related experience.
• Must reside in Hillsborough County, Florida.
• Must have a valid state driver’s license and maintain personal vehicle liability insurance.
• Must have a separate room with a locked door for use as a private home office.
• Must complete tuberculosis screening because the role is patient-facing.

Nice to have

• Experience documenting electronic case notes across multiple computer applications and systems.
• Experience working with Medicare and Medicaid recipients.
• Experience in health promotion, coaching, and wellness.
• Knowledge of community health, social service agencies, and other community resources.
• Bilingual communication skills.
