TL;DR
Enrollment Specialist (Healthcare): Managing end-to-end enrollment for prospective PACE participants by coordinating applications, documentation, communications, eligibility activities, and enrollment visits with an accent on participant-centered care, regulatory compliance, and cross-functional coordination. Focus on removing enrollment barriers, guiding participants and caregivers through complex decisions, and using performance data to improve the enrollment experience.

Location: San Leandro, CA, United States; travel throughout the service area and visits to participants' homes, centers, community locations, or virtually may be required.

Salary: $74,640–$79,000 annually, plus a target monthly bonus and potential additional incentive compensation.

What you will do

  • Manage the end-to-end enrollment journey for prospective participants as the enrollment case manager.
  • Review and update applications, required documentation, enrollment materials, communications, and follow-up activities.
  • Conduct enrollment visits in participants' homes, centers, or virtually to assess needs and support eligibility activities.
  • Partner with Outreach, the Enrollment RN, the Interdisciplinary Team, and center leaders to coordinate enrollment.
  • Identify and resolve enrollment barriers while building trusted relationships with participants and caregivers.
  • Monitor enrollment metrics, maintain accurate records, and support outreach and referral development.

Requirements

  • Bachelor's degree or equivalent combination of education and relevant experience.
  • At least one year of experience in healthcare sales, enrollment, customer success, business development, care navigation, or a similar relationship-focused role.
  • Strong communication, organization, collaboration, empathy, and decision-making skills.
  • Experience with Microsoft Office and ability to learn CRM, enrollment management, and participant relationship management systems.
  • Valid driver's license, reliable personal transportation, qualifying automobile insurance, and ability to travel throughout the service area.
  • Active CPR/BLS certification or ability to obtain it within 30 days; compliance with participant-facing health, vaccination, infection-control, and PPE requirements.

Nice to have

  • Experience in PACE, Medicare, Medicaid, managed care, or other value-based healthcare environments.
  • Knowledge of PACE, CMS, and enrollment, eligibility, and care-coordination regulations.
  • Experience with CRM or enrollment management systems.
  • Bilingual Spanish, Mandarin, or Cantonese.
