CCM Case Management RN

Location: Remote for candidates residing in Arizona, Florida, Georgia, or Texas; occasional travel may be required for team meetings and company events.

Salary: $32.60–$42.79 per hour

What you will do

• Coordinate care across emergency, inpatient, post-acute, outpatient, and home-care settings.
• Conduct comprehensive biopsychosocial needs assessments and develop outcome-based care plans.
• Support safe discharge planning and help members navigate complex healthcare situations.
• Advocate for members by addressing barriers to care and consulting medical directors when needed.
• Educate members and internal teams about health conditions, medications, procedures, and the CCM program.
• Participate in surveys, studies, process improvement initiatives, pilot programs, and special projects.

Requirements

• Active, unrestricted RN license in the state of hire, with the ability to obtain additional licenses as needed.
• At least 2 years of clinical experience.
• Ability to communicate with members by phone, secure messaging, and in person.
• Ability to work remotely from one of the listed states.

Nice to have

• Working knowledge of Milliman Care Guidelines.
• Case management experience or CCM certification.
• BSN.
• Previous work-from-home experience and comfort accessing systems remotely.
• Bilingual Spanish or Creole skills.
