Care Coach (Telehealth)

Location: Remote within AZ, DC, DE, FL, GA, HI, IL, KS, MA, MD, ME, MI, MN, MO, NC, NJ, NM, NV, NY, OH, OR, PA, SC, TN, TX, VA, VT, or WI. The role supports shifts covering 24/7/365 care delivery, including Tuesday–Saturday and Sunday–Thursday schedules in EST.

Salary: $62,000–$73,000 per year, plus equity and benefits.

What you will do

• Drive member engagement, program opt-in, goal setting, adherence, and sustained behavior change.
• Manage a caseload through messaging, video appointments, phone outreach, and ongoing follow-up.
• Apply motivational interviewing, behavior modification, health education, and clinical program protocols.
• Monitor member health data and platform activity to identify risks and take appropriate action.
• Coordinate care across providers, digital networks, in-person provider networks, and external partners.
• Collaborate with care teams and contribute to quality improvement and clinical outcome initiatives.

Requirements

• 1–2+ years of experience in health coaching or clinical work, including patient education and motivational interviewing.
• Experience with maternity, family planning, fertility care, or coaching toward specific health outcomes.
• Experience tracking outcome-related KPIs such as opt-in rates, follow-up adherence, and member satisfaction.
• Strong written and verbal communication, empathy, flexibility, and ability to adapt in a fast-changing environment.
• Telehealth competency, including phone and video outreach, plus reliable high-speed internet.
• Active US work authorization and residence in one of the listed states are required.

Nice to have

• NBC-HWC or ICF health coaching certification.
• Startup or fast-paced environment experience.
• Experience coaching through video, text/chat, and phone.
• Experience with Google Suite, CRM platforms, EHRs, or Zendesk.
