Care Coach (Healthcare)

Location: Remote within the US; open to candidates in AZ, DC, DE, FL, GA, HI, IL, KS, MA, MD, ME, MI, MN, MO, NC, NJ, NM, NV, NY, OH, OR, PA, SC, TN, TX, VA, VT, and WI. Active US work authorization required. Shifts include Tuesday–Saturday or Sunday–Thursday coverage during EST hours.

Salary: $62,000–$73,000 per year, plus equity and benefits.

What you will do

• Drive member engagement, program enrollment, behavior change, and health outcomes.
• Co-create SMART goals, track adherence, adjust care plans, and course-correct within clinical program guidelines.
• Manage a caseload through messaging, video appointments, phone outreach, and ongoing follow-up.
• Provide personalized health education using motivational interviewing and behavior modification techniques.
• Monitor member health data and platform activity to identify risks and take action according to established protocols.
• Coordinate care with providers, digital and in-person networks, and external partners while contributing to quality improvement.

Requirements

• 1–2+ years of experience in health coaching or clinical work, such as nursing or behavioral health.
• Experience with patient education, motivational interviewing, health-outcome coaching, and KPI tracking.
• Experience supporting maternity, family planning, or fertility care populations.
• Strong written and verbal communication skills with an empathetic, adaptable approach.
• Strong telehealth skills, including phone outreach and video calls, with reliable high-speed internet.
• Comfort using digital health tools, CRM platforms, EHRs, Zendesk, Google Suite, and AI tools in daily workflows.

Nice to have

• Health coaching certification such as NBC-HWC or ICF.
• Experience in a startup or fast-paced environment.
• Experience coaching through video, text or chat, and phone modalities.
