Home Care Coordinator (LVN)

Location: Compton, California, United States. The role involves in-home and community work, travel between work sites as needed, and communication in person, by phone, and through electronic systems.

Salary: $29–$36 per hour, with eligibility for an annual bonus and potential additional incentive compensation.

What you will do

• Conduct comprehensive in-home assessments covering care needs, functional status, safety risks, and suitability of home care services.
• Perform home safety evaluations, assess the use of adaptive equipment, and recommend measures that promote independence and reduce risk.
• Collaborate with participants, caregivers, families, rehabilitation, nutrition, social services, and other interdisciplinary team members.
• Participate in developing, implementing, and updating individualized participant care plans.
• Coordinate home care services, review caregiver documentation, identify missed or declined services, and follow up on changes in participant condition and service gaps.
• Maintain service authorizations, coordinate renewals, support quality improvement and compliance activities, and keep accurate electronic health records.

Requirements

• At least two years of clinical experience, preferably in home care, geriatrics, care coordination, primary care, or a related healthcare setting.
• Knowledge of care planning, clinical assessments, home care delivery, and identifying changes in condition or care needs.
• Strong clinical judgment, critical thinking, organization, communication, collaboration, and ability to manage multiple priorities.
• Proficiency with electronic health records and healthcare documentation systems.
• Valid driver's license, reliable transportation, and active auto insurance meeting state requirements.
• Ability to use motivational interviewing, trust-building techniques, and clear expectation setting to support appropriate home care recommendations.

Nice to have

• Active LVN license in the applicable state.
• Experience with older adults, PACE, home health, home care, managed care, care coordination, utilization management, or authorization processes.
• Bilingual Spanish, Mandarin, or Cantonese skills.
