Clinical Guide Population Health RN

Location: Denver, Colorado; hybrid schedule with remote work on Monday, Wednesday, and Friday and in-office work on Tuesday and Thursday. Shift: 9:00 am–5:30 pm MST. Occasional holiday and weekend coverage may be required.

Salary: $34.00–$40.86 per hour, based on geographic location, experience, and qualifications.

What you will do

• Perform clinical triage using evidence-based protocols and algorithms to direct members to appropriate care resources.
• Conduct proactive outreach to high-risk, high-spend, rising-risk, and care-gap members using data-driven analytics.
• Assess member needs, risks, preferences, and barriers while providing clinical guidance, condition education, and lifestyle support.
• Coordinate appointments, referrals, screenings, follow-up care, chronic disease support, and medication adherence.
• Document symptoms, nursing assessments, guidance provided, and member responses accurately.
• Collaborate with the clinical lead and Navigation team while advocating for members and staying within RN scope of practice.

Requirements

• RN or BSN degree.
• Active RN license in a compact state and residence in a compact state.
• 5+ years of direct patient care experience, preferably in family medicine.
• Experience in population health, disease management, or a similar field.
• Triage nursing experience is desired.
• Care management or chronic condition support experience is desired.

Nice to have

• Multiple RN state licenses, particularly in California, New York, Michigan, or Massachusetts.
• Experience in family medicine, triage nursing, care management, or chronic condition support.
