Case Management RN

Location: Remote within Arizona, Florida, Georgia, or Texas; occasional travel may be required for team meetings and company events.

Salary: $32.60–$42.79 per hour

What you will do

• Coordinate member care across inpatient, outpatient, post-acute, emergency, and home-care settings.
• Support members facing complex health challenges and develop individualized care plans.
• Work with hospital case managers on discharge planning and care transitions.
• Educate members about health conditions, medications, and procedures through phone and secure messaging.
• Collaborate with support teams on member care, process improvement, and pilot programs.
• Ensure compliance with applicable laws and regulations.

Requirements

• Active, unrestricted US RN license in the applicable state or an active compact multistate unrestricted RN license.
• Ability to obtain additional state licenses as needed.
• At least 2 years of clinical experience in hospital, outpatient, or community-based care management.
• At least 1 year of experience in care coordination and navigation.

Nice to have

• BSN.
• Working knowledge of Milliman Guidelines.
• CCM certification.
• Behavioral health experience.
