Care Coordinator

Location: Remote Illinois; applicants must reside in Cook County within the specified ZIP codes or within a 10-mile radius. This is a field-based role with 75–90% local travel and occasional meetings at the Schaumburg, Illinois office.

Salary: $59,100–$79,900 per year.

What you will do

• Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings.
• Assess care needs and ensure members receive services in the least restrictive setting.
• Identify and resolve barriers that affect effective care.
• Develop, implement, monitor, and evaluate individualized care plans and interventions.
• Coordinate services with providers, payer sources, interdisciplinary teams, families, and community partners.
• Track member progress toward personal health and well-being goals.

Requirements

• Must reside in Cook County, Illinois, within one of the listed ZIP codes or within a 10-mile radius.
• Bachelor’s degree in social sciences, social work, human services, or a related field, or an active unrestricted Illinois LPN license with at least one year of relevant assessment and service-provision experience.
• At least one year of healthcare or case management experience.
• At least one year of experience with Medicare or Medicaid recipients, long-term care, HCBS, or managed care organizations.
• Ability to use electronic information systems, including electronic medical records, and travel throughout the region for face-to-face interactions.
• Valid state driver’s license and personal vehicle liability insurance are required.

Nice to have

• Active unrestricted Illinois LPC or LSW license.
• Bilingual or multilingual ability in English and Spanish, Arabic, Vietnamese, Amharic, Urdu, or another language.
