Transition/Custodial Prevention Specialist (Healthcare)

Location: Remote in Illinois, with 75–90% regional field travel to meet members and occasional onsite meetings in Schaumburg, Illinois. Work hours are Monday–Friday, 8:00 AM–5:00 PM Central Standard Time, with some schedule flexibility.

Salary: $59,300–$80,900 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess member goals, needs, barriers, resources, and service eligibility through telephone and face-to-face interactions.
• Develop and update individualized service and care plans with members, caregivers, primary care providers, and interdisciplinary teams.
• Coordinate long-term services and supports, home and community-based services, behavioral health resources, housing, education, and other community services.
• Support nursing facility residents, advocate for needed care, and assess readiness and ability to return to the community.
• Build relationships with facility staff, providers, payer sources, community partners, and care teams.
• Educate members about maintaining Medicaid eligibility and enter annual level-of-care assessments into the state portal.

Requirements

• Must reside in Illinois.
• Bachelor’s degree in social sciences, social work, human services, or a related field.
• At least two years of experience with Medicare and Medicaid recipients, long-term care services, home and community-based services, or managed care organizations.
• At least one year of professional experience in custodial and transitional care.
• Experience with electronic information systems, including electronic medical records.
• Ability to travel throughout the region to meet members, families, community partners, and care teams; a valid state driver’s license and personal vehicle liability insurance are required.

Nice to have

• Active, unrestricted Illinois LPC, LSW, or RN license.
• Nursing home diversion, long-term care case management, or waiver program experience.
• Experience with complex health populations, health promotion, coaching, and wellness.
• Knowledge of community health and social service agencies.
• Spanish, Arabic, Vietnamese, Amharic, Urdu, or another additional language.
