Social Worker (PACE)

Location: Onsite in Sacramento, California, United States

Salary: $38–$45 per hour, with possible additional incentive compensation.

hirify.global provides integrated medical and social care for older adults through the Program of All-Inclusive Care for the Elderly (PACE), supporting participants in centers and at home.

What you will do

• Collaborate with the Interdisciplinary Team to optimize participants’ health status, quality of life, and care plans.
• Conduct social work assessments to identify psychosocial needs, preferences, and goals.
• Deliver and document interventions, including resource coordination, care transitions, individual and group counseling, and case management.
• Coordinate with primary care providers and care team members during transitions between hospitals, skilled nursing facilities, homes, and other settings.
• Initiate and facilitate care conferences with participants, families, caregivers, and other support-network members.
• Provide discharge planning when participants disenroll from the program.

Requirements

• Master’s degree in Social Work (MSW).
• At least one year of experience working with frail or elderly people.
• Experience coordinating and facilitating care conference meetings.
• Valid California driver’s license, personal transportation, a good driving record, and auto insurance required.
• Medical clearance, TB testing, current immunizations, and compliance with applicable vaccination, infection-control, and PPE policies required.
• Current CPR/BLS certification, or the ability to obtain it within 30 days of employment.

Nice to have

• Bilingual Spanish and English skills.
• Experience supporting patients or participants with behavioral health and substance abuse issues.
• Experience with PACE, ADHC, or CBAS programs.
