Bilingual Non-Clinical Care Coach

Location: Remote Puerto Rico; must reside in Puerto Rico. Scheduled hours are 8:30 AM–5:30 PM EST, or 9:30 AM–6:30 PM during Daylight Savings. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $43,000–$58,100 per year

What you will do

• Conduct primarily telephonic outreach to members with multiple chronic conditions and financial or functional barriers.
• Review member data, perform structured assessments, and develop and manage individualized care plans.
• Coordinate services and connect members with clinical, community, and social-service resources.
• Identify and resolve barriers affecting effective care and member independence.
• Monitor health outcomes and adjust care strategies as needed.
• Maintain compliance while working across multiple computer systems in a structured environment.

Requirements

• Must reside in Puerto Rico.
• Associate degree in any subject; ADN, RN, and LPN degrees do not qualify.
• Bilingual English and Spanish proficiency in reading, writing, and speaking.
• At least two years of direct adult patient-care experience.
• Experience working with healthcare teams and building relationships with internal and external customers.
• Experience using electronic medical records and other software systems.

Nice to have

• Bachelor's degree in Allied Health Science.
• Experience with elderly or chronically ill patients, Medicare, or Medicaid.
• Experience conducting health assessments, coordinating care, or assisting with health benefits.
• Knowledge of health promotion, wellness coaching, community health, and social-service resources.
