Case Manager- Washburn County

Location: Washburn County, Wisconsin and surrounding areas; home-based work when not conducting member visits. Travel up to 40% and occasional travel to hirify.global offices may be required. Hours are Monday-Friday, 8:00 am-4:30 pm CST.

Salary: $53,700-$72,600 per year

What you will do

• Assess members’ health, safety, strengths, interests, and preferences to develop comprehensive Member Care Plans.
• Coordinate with Field Care Nurses and arrange services in the least restrictive environment.
• Conduct enrollment and recurring face-to-face social assessments, quarterly in-person visits, and monthly phone contact.
• Arrange social integration, community resource, employment, housing, and other non-medical support services.
• Evaluate risk factors, educate members, and ensure cost-effective service delivery.
• Maintain case notes, service authorizations, and Member Care Plan updates.

Requirements

• Bachelor’s degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor’s degree in another field plus at least three years of relevant experience.
• Valid state driver’s license and personal vehicle liability insurance with minimum limits of 100/300/100.
• Ability to conduct patient-facing home visits and travel throughout Washburn County and surrounding areas.
• Ability to work from a dedicated home space that protects member PHI and HIPAA information.
• Minimum home internet speeds of 25 Mbps download and 10 Mbps upload.
• Completion of TB screening is required for the patient-facing role.

• Residence within 45 minutes of the assigned coverage area.
