Case Manager (Healthcare)

Location: Remote within the United States, with coverage of Sheboygan, Wisconsin and the surrounding area; travel to members’ homes is required for face-to-face visits, with up to 40% travel and occasional office travel. Working hours are Monday–Friday, 8:00 am–4:30 pm CST.

Salary: $53,700–$72,600 per year.

What you will do

• Assess member health, safety, strengths, interests, and preferences to create comprehensive Member Care Plans.
• Coordinate with Field Care Nurses and arrange services that support members in the least restrictive environment.
• Conduct enrollment and recurring face-to-face social assessments and quarterly in-person home visits.
• Maintain monthly phone contact and coordinate community, employment, housing, social integration, and other non-medical services.
• Evaluate risks, educate members, ensure cost-effective service delivery, and maintain case notes, authorizations, and care plan updates.

Requirements

• Bachelor’s degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor’s degree in another field plus at least three years of relevant experience.
• Ability to travel up to 40% for member visits within the Sheboygan coverage area.
• Valid state driver’s license and personal vehicle liability insurance with 100/300/100 limits.
• Ability to work Monday–Friday, 8:00 am–4:30 pm CST and maintain secure documentation practices for member PHI and HIPAA information.
• Ability to work from a dedicated home workspace with internet speeds of at least 25 Mbps download and 10 Mbps upload.
