Physical Therapist, Part Time

Location: 855 Howe Avenue, Sacramento, California; services are delivered at the Sacramento PACE Center and participant homes.

Salary: $47.00–$71.00 per hour

What you will do

• Evaluate and treat PACE participants across acute, restorative, and maintenance levels of care.
• Create individualized treatment plans and educate participants, caregivers, and families on exercise programs and durable medical equipment.
• Conduct home visits as needed and document therapy services, participant responses, and care activities.
• Participate in the Interdisciplinary Team through clinical assessments, care-plan recommendations, and care coordination.
• Support physical therapy program development, group maintenance exercises, quality audits, data collection, and DME inventory management.
• Maintain documentation, safety, environmental, and infection-control standards.

Requirements

• Bachelor’s degree in physical therapy and completion of an accredited physical therapy program.
• Active California Physical Therapist license required.
• At least one year of experience working with frail or elderly patients.
• Current CPR/BLS certification.
• Ability to adapt in a fast-paced, evolving environment and contribute to continuous improvement.
• Employment requires authorization to work in the United States; hirify.global participates in E-Verify.

Nice to have

• Two or more years of experience as a practicing physical therapist.
• Strongly preferred: bilingual Spanish/English; additional languages are a plus.
• PACE and/or ADCH/CBAS experience.
• State-issued driver’s license, personal transportation, and legally required auto insurance.
