Home Care Coordinator (PACE)

Location: Sacramento, CA, United States; travel between work sites and community locations is required as needed.

Salary: $29.00–$36.00 per hour, plus a target annual bonus of 5% and possible additional incentive compensation.

What you will do

• Conduct comprehensive in-home assessments covering care needs, functional status, safety risks, and suitability of home care services.
• Perform home safety evaluations, assess the use of adaptive equipment, and recommend measures that support independence and reduce risk.
• Collaborate with participants, caregivers, families, and interdisciplinary team members to identify needs, service gaps, and changes in condition.
• Develop, implement, and update individualized care plans as an active member of the Interdisciplinary Team.
• Coordinate home care services, review caregiver documentation, monitor missed or declined services, and follow up on concerns, grievances, and Service Determination Requests.
• Maintain service authorizations, coordinate renewals, complete compliant electronic health records documentation, and support quality improvement activities.

Requirements

• At least two years of clinical experience, preferably in home care, geriatrics, care coordination, primary care, or a related healthcare setting.
• Knowledge of care planning, clinical assessments, home care service delivery, and healthcare documentation systems.
• Strong clinical judgment, critical thinking, organization, communication, collaboration, and ability to manage multiple priorities.
• Ability to work effectively within an interdisciplinary team and use motivational interviewing, trust-building techniques, and clear expectation setting.
• Valid driver's license, reliable transportation, and active auto insurance meeting state requirements.
• Must be authorized to work in the United States; hirify.global participates in E-Verify.

Nice to have

• Active Licensed Vocational Nurse (LVN) license in the applicable state.
• Experience with older adults, PACE, home health, home care, managed care, care coordination, utilization management, or authorization processes.
• Spanish, Mandarin, or Cantonese bilingual ability.
