Health Home Care Manager (Healthcare)

Location: Field-based in Chautauqua County, New York, with frequent travel to patients’ homes in Buffalo and surrounding areas within Erie County

Salary: $24–$26 per hour

What you will do

• Outreach to potentially eligible patients, obtain consent, and enroll them in the Health Home Care Management Program.
• Conduct eligibility screenings, comprehensive assessments, reassessments, home visits, and patient follow-up.
• Develop and coordinate patient-centered care plans for patients with chronic medical, mental health, and substance use conditions.
• Connect patients with healthcare providers, pharmacies, insurers, hospitals, family caregivers, and local community resources.
• Provide direct support, address access barriers, intervene in crises, and document services and outcomes according to Health Home standards.
• Maintain patient confidentiality while managing a high-volume caseload.

Requirements

• Ability to travel in the field, conduct home visits, and meet patients in person.
• At least one year of Health Homes, HARP, or case management experience.
• Knowledge of local community resources and experience with data reporting, documentation, and outcomes.
• Strong communication, assessment, empathy, and crisis-management skills.
• Ability to manage a high-volume caseload and navigate multiple data management systems, including Salesforce.
• Associate degree required; bachelor's degree preferred.

Nice to have

• Bilingual Spanish language skills.
• Master's degree.
• Three or more years of Health Homes or HARP experience.
