Health Home Care Manager (Healthcare)

Location: Field-based in Buffalo, NY and surrounding areas within Erie County; frequent travel to patients’ homes is required.

Salary: $24.00–$26.00 per hour

What you will do

• Outreach to potentially eligible patients, obtain consent, and enroll them in the Health Home Care Management Program.
• Conduct functional eligibility screenings, initial assessments, reassessments, home visits, and patient follow-up.
• Develop and coordinate patient-centered care plans for patients with chronic medical, mental health, and substance use conditions.
• Connect patients with healthcare providers, pharmacies, insurers, community resources, family caregivers, and hospital discharge services.
• Provide direct support, facilitate referrals, address access barriers, and intervene in crisis situations.
• Document services, care plan outcomes, and patient contacts while maintaining confidentiality and meeting Health Homes standards.

Requirements

• Ability to travel in the field and meet patients in person is required.
• At least one year of Health Homes, HARP, or case management experience.
• Knowledge of local community resources and ability to manage a high-volume caseload.
• Strong communication, assessment, empathy, documentation, and data-reporting skills.
• Ability to work with patients, families, medical providers, and community care partners during rapidly changing situations.
• Minimum associate’s degree; a bachelor’s degree is preferred.

Nice to have

• Bilingual Spanish skills.
• Master’s degree.
• Three or more years of Health Homes or HARP experience.
• Experience with data management systems such as Salesforce.
