Community Health Partner (Healthcare)

Location: Raleigh, NC, United States; member visits may take place in homes, shelters, hospitals, and other community settings.

Salary: $27.40–$35.10 per hour

What you will do

• Collaborate with RN Care Managers and interdisciplinary care teams to support members with complex medical and behavioral health needs.
• Manage a member caseload and identify social, behavioral, and environmental barriers to optimal health.
• Develop and update care plans with members, supporting progress toward social and health-related goals.
• Conduct self-efficacy and condition-specific assessments, including PHQ-9, GAD-7, AUDIT, and DAST-10 screenings.
• Coordinate provider appointments, diagnostic and laboratory testing, behavioral health referrals, and community resources.
• Document interactions, track tasks, communicate with care teams, and escalate members who need additional support.

Requirements

• 2+ years of progressive experience in care coordination, community health, health promotion, or a related field.
• Strong critical-thinking, judgment, problem-solving, and escalation skills.
• High-quality written, verbal, and presentation communication skills.
• Previous experience with documentation and care coordination workflows.
• Ability to meet members in community settings in the Raleigh area as needed.
