Health Home Care Manager (Healthcare)

Location: Rochester, NY and surrounding areas; field-based with frequent travel to patients' homes

Salary: $24.00–$26.00 per hour

What you will do

• Outreach to potentially eligible patients, obtain consent, and enroll them in the Health Home Care Management Program.
• Conduct functional eligibility screenings, comprehensive assessments, and reassessments covering medical, mental health, substance use, financial, housing, and support needs.
• Develop, implement, and coordinate patient-centered care plans for patients with chronic conditions.
• Connect patients with referrals, community resources, healthcare providers, pharmacies, insurers, hospitals, and family caregivers.
• Conduct home visits, provide crisis intervention, maintain patient contact, and document services according to Health Homes standards.
• Protect patient confidentiality and track care plan outcomes across data management systems.

Requirements

• Ability to travel in the field and meet patients in person in Rochester, NY and surrounding areas.
• At least 1 year of Health Homes, HARP, or case management experience.
• Strong knowledge of local community resources and experience with data reporting, documentation, and outcomes.
• Strong communication, assessment, empathy, and crisis-management skills.
• Ability to manage a high-volume caseload and work with patients, families, and community care providers.
• Minimum associate's degree; bachelor's degree preferred.

Nice to have

• Bilingual Spanish proficiency.
• Master's degree.
• At least 3 years of Health Homes or HARP experience.
• Experience with Salesforce and other data management systems.
