Health Home Care Manager (Healthcare)

Location: Field-based in Buffalo, New York, with frequent travel to patients' homes and in-person appointments

Salary: $24.00–$26.00 per hour

What you will do

• Outreach to potentially eligible patients, obtain consent, and enroll them in the Health Home Care Management Program.
• Conduct functional eligibility screenings, initial assessments, reassessments, and comprehensive evaluations of medical, behavioral health, financial, housing, and support needs.
• Develop, implement, and coordinate patient-centered care plans for patients with chronic conditions.
• Facilitate referrals, resolve access barriers, connect patients with community resources, and coordinate services with providers, pharmacies, insurers, hospitals, and family caregivers.
• Provide direct support, crisis intervention, home visits, and ongoing patient contact while documenting services and outcomes.
• Maintain patient confidentiality and ensure compliance with Health Home standards.

Requirements

• Associate's degree required.
• At least 2 years of Care Management experience.
• Ability to travel in the field and meet patients in person in Buffalo, New York.
• Knowledge of local community resources and experience managing high-volume caseloads.
• Strong communication, assessment, documentation, data reporting, empathy, and crisis-management skills.
• Experience working with multiple data management systems, such as Salesforce.
