Care Manager, Health Homes

Location: Field-based in Bronx, New York, with frequent travel to patients' homes and in-person appointments

Salary: $24.00–$26.00 per hour

What you will do

• Conduct outreach, determine Health Home eligibility, obtain enrollment consents, and complete initial and ongoing assessments.
• Develop, implement, and coordinate patient-centered care plans for patients with chronic medical, mental health, and substance use conditions.
• Manage a caseload of chronically ill patients by facilitating referrals, resolving access issues, and connecting patients with community resources.
• Coordinate services with medical providers, pharmacies, insurance companies, hospitals, family caregivers, and other care providers.
• Conduct home visits, provide crisis intervention when needed, and maintain accurate documentation in accordance with Health Home standards.
• Protect patient confidentiality and document care plan outcomes.

Requirements

• Associate's degree required.
• At least 2 years of Care Management experience.
• Ability to travel in the field and meet patients in person in the Bronx.
• Knowledge of local community resources and experience handling high-volume caseloads.
• Strong communication, assessment, empathy, documentation, data reporting, and crisis management skills.
• Experience working with multiple data management systems, such as Salesforce.
