Care Manager Telephonic Nurse 2 (Healthcare)

Location: Remote Illinois; occasional travel to hirify.global offices may be required for training or meetings. Working hours are between 8:00 a.m. and 7:00 p.m. Central Standard Time, with core hours primarily from 8:00 a.m. to 5:00 p.m.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members' physical, environmental, and psychosocial health needs by phone.
• Develop care plans and monitor progress toward desired health outcomes.
• Provide education on chronic conditions and gaps in members' healthcare plans.
• Identify and resolve barriers to effective care and connect members with appropriate resources.
• Document assessments, conversations, care plans, and outcomes across electronic systems.

Requirements

• Bachelor's degree in a health-related field.
• 3+ years of clinical acute care experience.
• Active Illinois Registered Nurse license with no disciplinary action and the ability to obtain Illinois RN licensure.
• Ability to work 40 hours per week during the stated Central Standard Time schedule.
• Dedicated home workspace and internet service with at least 25 Mbps download and 10 Mbps upload speeds to protect member PHI and HIPAA information.

Nice to have

• Bilingual English/Spanish, English/Polish, or English/Creole skills, including speaking, reading, and writing without limitations.
• Certified Case Manager certification.
• Case management, discharge planning, adult acute care patient education, or managed care experience.
• Experience with Medicare and Medicaid recipients, health promotion, coaching, and wellness.
• Knowledge of community health, social service agencies, and community resources.
